POSITION TITLE: Nurse Care Manager

Position Summary:

Administrative responsibility for the day-to-day provision of Behavioral Health Care services. Plan develop and implement new programs and services. Ensure continuity of services to clients with aim of maximizing levels of functioning. Provide direct service as needed..

Position Qualifications:

  • NJ license to practice as a RN
  • Possess knowledge of evidence-based practices (evaluation, treatment, prescribing medications, preventative wellness programs) in clinical preventive services to provide primary healthcare services for persons with serious mental illness
  • Documented experience with programs serving individuals with mental illness, substance abuse or other disenfranchised populations
  • Driver’s license

Required Certification/Licensure: Registered Nurse; CPR certification and BLS certification

Duties and Responsibilities:

Recruit; engage in treatment SMI consumers with significant health issues. Performs marketing activities in the community.

Consult with SMI consumers and family members to develop and monitor individualized service plans that outline medical service treatment plans, access to available wellness and recovery support activities and self-management plans

Coordinate care with healthcare home medical providers to ensure access to primary health care. May arrange for transportation and/or accompany consumer to appointments to ensure engagement.

Coordinate intensive nurse care management services for referred population with serious or chronic medical conditions.

Coordinate and ensure consumer access to the array of wellness and recovery education and self-help programs available to each agency and in the broader community.

POSITION TITLE: Outpatient Clinician -

Position Summary:

Responsible for providing evaluation, treatment planning and counseling to new and ongoing clients in individual, family or group therapy. Implement treatment objectives through coordination of services with other service providers.

Position Qualifications:

  • Masters degree in Social Work, Psychology, Counseling or related field
  • Minimum of two year mental health experience in direct service

Required Certification/Licensure: LSW, LCSW or LPC

CADC and/or Bilingual preferred

Duties and Responsibilities:

Determine current problems, mental status, family and personal history, treatment trecommendations and provisional diagnosis. Effectively engages a range of clients in therapeutic relationships toward the attainement of mutually agreeed upon goals

Develop and implement an Individual Recovery Plan (IRP) with each client at specified intervals, which prioritizes problems, goals, objectives and interventions in a time frame.Develop and manage a caseload

Provides case management and coordination services as needed

Maintain appropriate documentation according to agency standards and procedures.
