We also provide services for their family members. Our program includes:
- Behavioral Health Home
- Outpatient Services
- Partial Care Services
- Case Management Services and Supportive Housing
Open Access to Care program
For individuals seeking mental health care, every moment counts. Research shows that the longer an individual must wait for treatment, the less effective that treatment is. Although some individuals can afford to access private treatment immediately, most cannot and are often placed on waiting lists. Yet, several months spent waiting for treatment can have devastating affects on a person in need. AAMH Open Access to Care provides immediate mental health care when you need it. All you need to do is walk into our center and a Master's level mental health clinician will be able to provide a psychosocial evaluation. A personalized, confidential plan for treatment will be developed and begin that moment.
Behavioral Health Home
Our Behavioral Health Home integrates behavioral health and primary care utilizing a holistic approach to wellness. The program coordinates clients medical and mental health needs and provides individualized support to help clients obtain and follow through with all aspects of treatment. BHH promotes healthy lifestyles to maintain or prevent chronic medical illnesses. All services are provided by a highly dedicated, multi-disciplinary team of Licensed and Registered Nurses, social workers, medical assistants and other specialty coaches.
Child and Adult Outpatient Services
All our clients are given personalized, confidential counseling by Master's level Social Workers and Counselors. Our psychiatric staff provides evaluation, consultation and medication management services if needed. Our immediate goal is to improve or stabilize our clients' functioning and well-being. Then we work closely with them on their long-term needs and goals
Partial Care Program
Partial Care is a wellness-based, day-treatment program that provides an integrated approach to mental health and substance abuse care. With the help of their personal counselor, clients develop their own individualized recovery plan based on their values and wants, including returning to work or school. Families are encouraged to be part of the planning process. Clients can chose to participate in a variety of groups, which includes addiction, depression/anxiety support, IMR, socialization, vocational and health & wellness. Board certified psychiatrists and Psychiatric Advanced Practice Nurses provide psychiatric evaluation, medication monitoring and psychiatric consultation. Breakfast and lunch is available daily. Extra help and support are offered clients and their families in the bi-monthly education and support group. Addiction and/or vocational tracks are available
Case Management Services and Supportive Housing
AAMH provides support to help clients sustain themselves in the community by providing independent housing through collaboration with Catholic Charities. This program continues the progress of our clients and links them to a network of care in Mercer County, which helps them find permanent housing and create long-term independent living opportunities.
